医局の功罪

前回の医局旅行のブログが予想外？に受けましたので、引き続いて医局の話題を続けたいと思います。

最近医局は厚生労働省や新聞から目の敵にされ、不正の温床といった捕らえられ方をされ、医局解体が叫ばれています。阪大は、白い巨塔の浪速大学と捉えられて、この手の話題になるといつも出てくるある意味代表選手？です。

ただ、私の個人的経験から言えば、医局は功罪両面があるように思いますし。主催する教授の性格にも大きく依存するように思います。

阪大はある意味都会型で、私が入局以降個人の意思が完全に無視して人事がきまったり、報復人事のようなケースはなかったように思います。むしろ互助会的側面が強かったように思います。

一方で、講演などにいって聞くケースでは、昔ながらの状態も確かにあるようでした。

しかし、地方での医師派遣は、医局がローテーションで過疎地域での勤務に対してある意味次の人事での見返りを約束することで確保されていた側面は見逃せません。

スーパーローテにより完全に医局崩壊が始まった今、どうやって地域への派遣医師に対するインセンティブを確保するか、これは大きな課題になってきています。

昔が良かったというつもりはありませんが、個々の医師の技術を上げたいという素朴な向上心による都会志向と過疎地域における医療の確保という現実の医療の確保をどうやって行うか？医局に代わる調整機能が必要だと思います。この回答はかなり難しいと思います。

　医局の同門会の案内状が届いた日に読むと…医局って悪い点ばかりじゃなかったよなぁって思います。ただ、今のように研修指定病院の方になびいてしまっている若手の医師を呼び戻すには「医師としてのキャリアパス」を大学がどれくらい提示できるかというのにかかってくるかなと思います。
　留学するにしても単身乗り込む方もいるわけですし、やはり大学ならでは…という切り口が欲しいです。そういう意味で先生のご活躍は大いに期待しております☆
