無借金経営を目指すのであれば、ハード面にはとにかくお金をかけることができない。
その分、医師個人の診療技術が重要になってくる。
ここ30年ほど、医者の診察より検査や薬に診療報酬がシフトしてしまったので、恐ろしいくらいに実は人間を診ることができなくなっているのではないか。
結果として起こることは、製薬会社の洗脳による、絆創膏処方である。
そしてそのターゲットは研修医だ。
現在の診療報酬では、技術や経験で診療報酬は変化がない。また地域差もない。
結果起こることは、早期開業である。開業権をとりあげるなんつう、医者の存在に関わることはそもそもいきなり改正することはできない。
できないから姑息な手段で、開業しても生業が成り立たなくしてしまうわけである。
設備投資を少なくしてでも開業しようという、人は出てくるだろう。
その最も便利な診療科が「精神科」だ。
なにしろ、最近では、スケールで「診断」できることになっているから。。
「お、あんたうつですね（おれ、内科医だけど＾＾）。だから抗うつ薬！」
「お、あんた物忘れですね、長谷川15点だから、だから、アリセ◎×！」
「お、人前に出るのがはずかしい？お、あんた、社会不安障害だね。はい、パキ◎×！」
不況であやしい「精神科」がますます増えるだろう。。

チェーン開業の業者は、「仲介業務」で、複数科診療で、各医師に借金を分散して、みためリスクを少なくすると言うパターンがほとんどではないでしょうか。
あるいは割高の家賃をとって、会社は痛まないようにする、いわば昨今話題になっているコンビニの店長問題といっしょだと思います。
こういう悪質な連中にお金をもっていかれる真面目な先生達がかわいそうです。
