An Alternative to Warfarin in Patients with Atrial Fibrillation?

心房細動（atrial fibrillation：AF）患者の脳卒中予防にwarfarinを使用することは困難を伴うことから、よりよい代わりの薬を見つけようとする取り組みがおこった。
まだFDAの承認を受けていない薬物であるdabigatran etexilateは、代謝されて、抗凝固のモニタリングを必要としない競合的に作用する直接トロンビン阻害物質になる。
製薬企業から支援を受けたこの試験では、脳卒中のリスクが高いAF患者18,113人（平均年齢71歳）を対象に、dabigatranがwarfarinに対して劣るかを確認することを主目的として、dabigatran（110mgまたは150mg、1日2回）とwarfarinを比較した。
フォローアップ期間の中央値は2年間で、この期間の被験薬の中止率は、dabigatran群はいずれの用量群も21％で、warfarin群では17％であった（P＜0.001）。
平均すると、warfarin群において国際標準比（international normalized ratio：INR）が治療域内を維持した期間は64％であった。
主要エンドポイント（脳卒中または全身性塞栓症）は1年あたり、低用量dabigatran群で1.53％の患者、高用量dabigatran群で1.11％の患者、warfarin群で1.69％の患者で生じ、両用量のdabigatranはwarfarinに対して非劣性であった。出血性脳卒中の発生率は、warfarin群（0.38％）が、dabigatran 110mg群（0.12％）、dabigatran 150mg群（0.10％）より有意に高かった。
大出血の発生率は、dabigatran 110mg群が他の2群より有意に低かった。
著者コメント：
この研究では、AF患者の脳卒中予防について、dabigatranの高用量、低用量ともに、warfarinに対して非劣性であった。
この結果は、dabigatranの投与の方が出血性脳卒中の発生頻度が低いこと、およびこの薬物が安全で効果的なwarfarinに代わる薬であることを示唆している。
dabigatranがFDAの承認を得た場合、米国の患者および保険支払機関は、その利便性（warfarinと比べての）と高くなりそうなコストを比較検討しなければならないだろう。
