読書感想『精神病院の社会史』

　高尾山は、江戸から甲府に抜ける最短ルート上にありますが、山道は険しく、修験道の道場でした。江戸時代に、表参道は一般の人がお参りをしたり富士登山（これもお参り）のために通行していましたが、山の裏側では、精神病者が長期逗留をして「水治療」を受けていました。滝のそばには旅館（一般人や症状の軽い病者用、明治時代には一日１円）や参籠所（精神病者や眼病者用、精神症状の重い人用に格子窓のついた隔離室もある、明治時代には１日38銭）がありました。療養目的の逗留は短くて３ヶ月、長いと数年ということもあったそうです。そこで、修行者に混じって、精神病者が滝に打たれて“治療”を受けていたのです。本書に載せられた宿帳の写真には、病名までちゃんと記載されています。
　1889年には甲武線が新宿〜立川〜八王子まで開通し、高尾山詣でが楽になります。1927年（昭和2年）には、大正天皇の多摩御陵ができ、当局から「聖域近くに、精神病者が野放し状態で参集するのはいかがなものか」とクレームがつきます。その影響か、同年小林病院（精神科、内科）が開設され、1935年にはその近くの佐藤旅館が高尾保養院となり、それまで荷物を運んだり滝に打たれる介助をしていた「強力」が「看護人」として病院に雇われます。病院になっても滝療法は継続されましたが、統合失調症の興奮に対しては卓効があったそうです。
　著者が高尾山の史料に夢中になって、焦点が安政年間よりあとに絞られてしまっているのが惜しまれます。たとえば『精神病院の起源』を読んでいれば、精神病者への「水治療」（滝に打たれる）が平安時代から主に密教系の寺院でおこなわれていたこと（漢方療法は室町時代頃から主に浄土真宗系の寺で、そして読経療法は江戸時代に日蓮宗系の寺で行なわれるようになった）に触れることで本書の記述がふくらみを増したでしょうに。

　お話変わって、東京府。東京府で最初に公認された精神病院は「加藤瘋癲病院」でした。ただ、病院の名前にも時代の反映があり、たとえば「加命堂病院」は1846年（弘化3年）「小松川狂疾治療所」から始まって「癲狂院」→「小松川精神病院」→「加命堂病院」→「加命堂脳病院」→「加命堂病院」と名前が変遷しています。「脳病院」とは今の感覚では古風な名前ですが、命名当時は「精神病は呪いや狐憑きなど超自然的な原因によるものではなくて、脳の器質的な異常によって起きる」という新しい概念を反映した時代の先端を行く名前だったことでしょう。
　1872年（明治5年）ロシア帝国アレクセイ大公が来日しました。それに合わせて東京では、こじき浮浪者240人の一斉狩り込みが行なわれ、彼らを継続収容するための施設として東京府頓狂院が1879年に設立、86年に巣鴨に移り89年（明治22年）に東京府立巣鴨病院と改名されました。
　1919年（大正8年）に施行された「精神病院法」で、内務大臣は各道府県に公立の精神病院の設置を命じることができますが、道府県にきちんとした精神病院を建築する余力がないという事情がある場合、「代用病院」として民間の精神病院を指定することができました。本来の「県立病院」の「代用」です。なお、その建築費の半分は国が補助しました。さらに大きなイベント（一番大きかったのは、天皇の行幸）があるとその地域には精神病院を建てる予算が下りやすかったそうです（戦後も、天皇陛下の御幸があるとなるとまず国道が舗装された、を思い出します）。ちなみに、上に書いた「加命堂脳病院」も1920年（大正9年）に東京府代用病院に指定されています。
　本書には面白い装置が紹介されています。ドイツで盛んに行なわれている「水治療器」Dampfdusche（ダンプドゥーシェ）です。本書の写真では、個室ブースのミストシャワー装置ですね。1932年の赤城山滝沢療養所ではこの装置と滝療法を売り物にしていたそうです。（ネットの画像では、http://shop.ms-company.de/index.php/cPath/262にいろいろ並んでいます）
　戦前に「精神病院」は約５万床になりましたが、終戦直後は5000床になっていました。理由は空襲による焼失と、軍の接収です。本土決戦で大量に発生するであろう傷病兵に備えるため陸軍が次々入院患者を追い出して陸軍病院として確保したのでした。追い出された人がどうなったか？　精神症状、空襲、食糧不足、結核の蔓延……確証はありませんが、悲惨な末路だったのではないかと想像はできます。それでも『灰色のバスがやってきた』（フランツ・ルツィウス）よりはマシと言えるのでしょうけれど（障害者を施設から連れ出すときのナチスの口実も「傷病兵のために空きベッドを作る」でした。ただし輸送の優先順位は、症状の重さとユダヤ人かどうか、で、連れ出された直後に次々「事故死」や「病死」の通知が家族に届いたのですが）。
　「衛戍病院」という耳慣れない言葉も登場します。私も知らなかったので国語辞典を引くと、「衛戍（えいじゅ）」とは「軍隊が一つの土地に長く駐屯すること」を意味し、衛戍病院は日本陸軍の衛戍地に建てられた病院のことです（1936年に「陸軍病院」と改名されました）。徴兵された人々はちょうど統合失調症の初発年代でもあり（公には否定されていましたが、戦争神経症もあったはずです）、初期対応をする衛戍病院精神科とその後方病院として継続治療を行なう精神病院は密接な関係があったはずです。ただ、その実態は明らかではありません。帝国の軍人が精神病院に入院するなんてこと自体が当時は「軍機」だったのでしょうね。
