　死語（74）吸入で喘息死

　1960年代だったか1970年代に、日本では「喘息患者が吸入薬で死んだ」と言われる例が続出したそうです。救急車が駆けつけてみたら、吸入薬のスプレーを握りしめて冷たくなっていた、といった報道があったそうで「吸入薬の使用　→　喘息死」の“方程式”ができあがったそうです。（ここまでは先輩の話からの過去の再構成）
　苦しいから吸入をする　→　軽い発作ならそれで楽になる。でも重症発作の場合気管が狭くなっているから薬が中に入らない　→　苦しいのが直らないから吸入を繰り返す　→　薬は中に入らない　→　どうしても楽にならないから救急車を呼ぶ　→　でも手遅れで発見される　という話の順序だったのではないか、と私は想像しています。なまじっか軽い発作なら楽になる吸入スプレーだったからそれにしがみついてしまって医者に行くべき時機を失してしまった、と。

「吸入薬と喘息死」
　ここにはイギリスの例が紹介されています。

　そのため一時喘息治療薬として吸入は人気を失いました。ところが「気管支喘息は気管支の病変。だったら薬が直接ターゲットに届く治療法が悪いわけがない（飲み薬や注射は、肝臓を通って代謝されたり全身にまず行ってしまって副作用を出しやすい）」という発想から吸入療法を粘り強く研究・実戦する人たちがいました。そのせいか、1980年代後半には吸入療法が復権します。「Daily Use（発作の有無に無関係に定期的に吸入を続ける）」と言う言葉を私はその頃知りました。

　今では喘息治療の第一選択はステロイドの吸入です。アレルギー学会ではこんな発表もあったそうです。
「吸入ステロイド使用頻度が高い都道府県では気管支喘息死亡率が低い」

　「ステロイド」と聞くとそれだけで頭から拒絶する人もおられるでしょうが、吸入の場合全身の副作用は出にくくしかも喘息の“現場”に直接届けられ、病気の本態である慢性炎症に対して効果的、と現状ではこれ以上は考えにくい治療法と私は思っています。
　……10年経ったらまた違うことを言っているかもしれませんが（苦笑）。

うちの子も吸入薬のおかげで助かりました。酷い発作だとサチュレーションも80くらいに落ちちゃうし、可哀相なくらい辛そうでした。
それが病院で吸入してもらうと嘘みたいに回復するんですよね。びっくりしました。
今は発作のない時もオノンやメプチン、テオドールを飲んでます。成長したせいかそれで治まってます。
中学生くらいになったらもっと体質改善されてないかなあと
期待しています。

小児喘息の多くは思春期には消えてくれる、と小児科医に教わったことがあります。体質改善というか、成長が大きく効くのではないか、と私は考えています。子どもの一番の「仕事」は「成長」ですから、本来の仕事をしてくれればいいわけですね。
