　ノロウイルス感染症

　新聞に時々「○○でノロ感染が大量発生」と小さく報じられているのを見ます。
　そういえば４年前でしたっけ、福山の老人施設でノロウイルス患者が施設内感染で大量に発生した時の報道の論調はすごかったですね。まるで『その施設が諸悪の根源」のような扱いで日本中にさらし者にされる雰囲気でした（O-157の時もはじめはそうでしたが）。今は発生してもさりげなく報じられていますが、「日本中にさらし者にしなければならない」くらいの重大事だったら、今でももっと大きく報じるべきではないか、と思います（O-157もね）。
　あ、報じられては困るかも。実は私の勤務する病院でも現在入院患者さんが一人ノロなのです。外泊をして帰院してから嘔吐下痢。明らかに外泊中の飲食が原因ですがこれもやはり扱いは「院内感染」なんですよねえ。もちろんきっちり糞口感染の“封じ込め”をやっています。

※院内感染防止の立場から最も重要なのは「アウトブレイク（ある地域あるいは一定の集団のなかで、ある一定の期間に予想以上の頻度で疾病が発生すること）の防止」です。そそっかしいマスコミなどは「院内感染」を単純に「院内に感染症患者が（多数）存在すること」と考えていますが、それは医療もことばも理解していない態度です。「アウトブレイクを起こさないための対策」だからこそ「院内感染予防」なのであって、「存在している感染症への対策」は「治療」です。院内感染の「感染」は、“名詞”ではなくて“動詞”として扱われるべきなのです。

　昔はノロウイルスという“もの（呼び名）”はありませんでした。冬になって発生する嘔吐下痢症は「お腹にくる風邪」とか「牡蛎にあたったかな」などと言われていました（そういえば「食中毒は夏」が当時の常識でしたっけ。なぜか牡蛎だけはその常識の例外でしたが）。やがてその一部が小型球形ウイルス（ＳＲＳＶ）によることがわかり、2002年にさらに詳しく分類されて「ノロウイルス」と命名されて現在に至っています。そういえば、あれほど日本中が大騒ぎをしていたのにノロウイルスの検査は保健適応が認められず、記憶はすでに不確かですがたしか自費で１万円とか２万円とかだったはず。しかも結果が出るのに１週間。その後安くて早い検査が出ましたが、それは正確性を犠牲にして速度を稼いでいたので、結果が「ノロではない」と出ても完全には信用できないものでした。（「感度」と「特異度」と言うと一般には馴染みがないことばですね。「偽陰性」を使ったらわかりやすいかしら）

　マスコミは「次は何が出るかな。どこをさらし者にしてやろうか』とわくわくしているだけではなくて、たまにはこういったことの過去の総括をやってくれませんかねえ。記憶を新たにするだけではなくて、知っていたつもりで実は、という新しい知見もあるはずです。
　あ、困るかな。過去に自分たちがいかにヒステリックに大騒ぎをしたか、も同時に明らかになってしまいますから。

私の勤務する施設で「嘔吐・下痢」で１４人の利用者が半隔離状態です。いまだ感染は拡大中です、職員にも一部広がっています。しかしこれと言う対策も取られないままに感染を見過ごしているとしか思えません、職員への教育・指導も中途半端です。こういうことで良いのか疑問です。でもマスコミにさらされるのも困ります。

　隔離の徹底とか手洗いとか次亜塩素酸ナトリウムであちこち拭くとか、技術的なことはすぐ思いつきますが、まずは経営者の“意識”ですよねえ。上が本気にならない限り全体は本気で動きませんから。（お金もかかりますし）
　「マスコミに漏れたら困りますよ」が経営者に上手く伝わるように経営者に近いスジに囁く手は使えません？　あるいは手っ取り早く、保健所に電話。いや、密告しろ、というのではなくて、「14人も嘔吐下痢が出て、原因がわからないんだけど、職員はどう対応したらいい？　アドバイスちょうだい」という“相談”です。保健所は慣れていますから、対応は丁寧ですよ（少なくとも私の経験では）。
