　医者の不養生（２）

　アメリカで男性医師を集めて研究している「Physicians' Health Study」（つまりは「人の健康を守る人」の健康はどんなものかの研究）ではいろいろ面白い（少量のアスピリンは脳梗塞の予防に役立つとかビタミンサプリが心血管障害を予防するのには役に立っていない、とかの）結果が公表されています。で、「アメリカの医師は他の職業に比べて心血管病のリスクが高い、とそこに書いてある」という文章をあるパンフレットで見つけて、本当かいな、と見に行ってみました（私は好奇心の塊です）。ただ、好奇心も英語力のなさには勝てず、とうとう当該の記載を見つけることはできませんでした。アメリカの男性医師にはずいぶん高率に心血管障害があるんだな、という感想は得ましたけれど。

　他のサイトもいろいろ検索して遊んでいたら、アメリカでの医師の平均余命は他の職種に比較してずいぶん短い、という記述がいくつか見つかりました。日本ではどうなんでしょう。私が自分の周辺をきょろきょろ見る限りでは、それほど短くも長くもない印象ですが……「健康の専門家」が集団として目立つほど長生きできない、ということは、やはり一種の「早死に」と言って良いのかもしれません。

　で、こんなことを言うと必ず愉快そうに「医者の不養生」と言う人が現れるのですが、医者が早死にするのがそんなに愉快ですか？

　ずっと昔、私がまだ研修医のころですが、胸部外科のベテランドクターが「夜中に電話がかかってくると、この年になっても心臓がびくんと飛び上がってしばらく不整脈が出たみたいになるんだ」と言っていたのを私は今でも覚えています。その人が不整脈発作を起したら、それも「医者の不養生」なんですかねえ。
　医者の生活にはストレスがつきまといます。「人の生き死に」に責任を感じるのは心理的に常に緊張状態にあるわけですし、物理的にも夜中だろうがなんだろうがじゃんじゃん電話がかかってくる（しかもその多くは緊急事態）なのはたしかに心臓に悪いものです。実際私も医者になってから本当に気の休まる時と言うのは持てません。以前は学会出張をしたらその間だけは解放されました（それでも会場から病棟に電話を入れたりしていました）が、今は携帯でいつでも呼び出される時代です。呼び出されなくても「待機」というだけで心のエンジンはいつでもフル回転できるようにアイドリング状態を保っています。（それでも、今勤務しているのは緊急事態が非常に少ない病院なので、「エンジン」はアイドリングを保っていても、以前の「（すぐ踏めるように）右足はアクセルペダルの上にずっと保った状態」ではなくて現在は「右足をペダルの脇に外した状態」になっていますけれど）
　私個人は、自分が選択した道ですからそのことについて自分から改めてとやかく言いたてようとは思いませんが、それでも他人に無責任に云々されるのはあまり嬉しくありません。自分で不摂生している医者（つまり本当に不養生をしている医者）だけを見るのではなくて、医者一般をつかまえて、いわく「聖職だから当然」、あるいはお気楽に「楽して稼げて、医者って、うらやましいなあ」、はては愉快そうに「医者の不養生」……


　医者は「自分が医師であること」のストレスだけで十分心血管病を起すリスクを高くしているわけですが、そこにさらに（医学以外の）人為的なストレス（たとえば濡れ衣訴訟リスクなど）を加えることで医者が死ぬリスクをさらに高めたい人がいます。「死ぬのは自分じゃない」から平気なのでしょうが、医者を医学に集中させないことでリスクを高めたい理由って、何なんでしょう？　早く殺したい？

　もちろんすべての医者が早く死ぬわけじゃないです。ストレスの処理が上手な人や図太い人は楽々と生き延びます。発病するかどうか・発病しても致命的なところがやられるかどうかは、確率の問題です。だけど、体が弱い医者やナイーブな医者は、早く死ぬ確率が高まります。死ぬ前に「これはたまらん」と辞める人も出るでしょう。ということは、ストレスをかければかけるほど、“弱い”医者は社会的に淘汰される（数が減る）ことになります。
　医者を責め立てている皆さん、医者を社会的に淘汰することで、図太い（他者からの評価とか人の思いなどに無神経な）医者が今より多数派をしめる医療の世界になることが本当にお望みで？　　で、その理由は？
