死語(15)疫痢

　子ども時代に読んだ本（『次郎物語』だったか『路傍の石』だったかな？）に子どもが疫痢で死ぬシーンが登場したとき、私はそれを読んでそれはそれは怖い思いをしました。漠然とした「成長」とか「未来」しか知らず、「自分が死ぬ」という概念を持っていない人間に「お前だっていつ死ぬかしれないのだ」という“現実”が突きつけられたのですから。昔はこの病気はけっこうポピュラーなものだったようです。そして「子どもが死ぬ」のもよくあることでした。

　良くわからないのが「赤痢」と「疫痢」の違いですが、赤痢はアメーバ赤痢や赤痢菌による病気で、疫痢はそれ以外の（死にいたる）急性胃腸感染症だったのでしょうか。急性胃腸感染症というとなんだかおどろどろしいものに見えますが、実際には腹痛と嘔吐下痢症です。「嘔吐下痢」で死ぬのか？は現代人の“常識”に基づく疑問で、点滴もきちんとできない時代状況だったら、子どもは大人と違ってあっさり脱水になりそのまま死んでしまうことがあります。

　医者になってしばらく私は「疫痢」ということばに接することはありませんでした。久しぶりにそれを見たのが、Ｏ157（おーいちごーなな）の騒動のときです。まるでこの世の終わりが来たような報道でしたっけ。で、その中に「昔疫痢と呼ばれていた病気の中に、このＯ157によるものが混じっていたのではないか」という記事がありました。タイムマシンがあったら確認できるのでしょうけれどね。（過去の病気の正体を知りたい、という点では、黒死病が本当にペストだったのか、とか、ナポレオンやモーツアルトの死因は、とか、中国の三国時代に流行した「傷寒」の正体は、とか、私は疑問をほかにもいくつも抱えています）

　そういえば、（いつもの）余談ですが、Ｏ157報道の過熱ぶりは、ものすごかったですね。最初期の当事者に話を聞いたことがありますが、やって来た報道陣の横暴ぶりは簡単に表現しがたいすごさだったそうです。そういえば、脳死移植の最初の数例のときの報道の過熱ぶりもすごかったですね。過熱といってもマスコミの「過」は結局一過性の「過」で、大騒ぎを自分たちで演出しておいて、飽きたらぽいで見向きもしません。Ｏ157だって、今は全然報道されませんが、無くなったわけじゃないですよ。
　毎日毎日日々新しく違うことに「これは大変だ」「こんどはこれが大変だ」と大騒ぎをしている人間がいたら、「どうせそれは今日“大変”なだけで、明日は別のことが大変になるんだろうな」と友人たちからの信頼を失うのがオチです。どうしてマスコミの信頼は失われないんだろう？　昨日大切なことは（状況が激変しない限り）今日も明日も大切なことではありませんか？

おかだ先生、いつも興味深い記事をありがとうございます。

今までずっと、疫痢とは、小児の赤痢と思っていました。
赤痢はたしか法定伝染病だったと思いますが、疫痢はどうだったでしょう？

アメーバ-赤痢と他の細菌と細菌毒によるもの、その全てが混同されていたのでしょうか？

死語といえば、昔は中気（脳梗塞？）で５年・１０年と自宅で寝たきりのお年寄りが凄く多かったです。
今はめったに無いですよね。
それこそリハビリの成果なのに・・。
これから一体どうなるのでしょう？

　私が小学校の保健体育で習った法定伝染病に赤痢はありましたが「疫痢」はありませんでした。医学部でも疫痢の授業はありませんでした。ということで「疫痢」という病気が単体で実在していたのかどうかに私は疑いを持っています。いろんな病気をきちんと鑑別診断せずにまとめて呼んでいたのではないかなあ、というのが私の推定です（確定ではありません……言い訳モード(^_^;)）。

　そうそう、法定伝染病も今は（人間に関しては）死語ですねえ。
