「マイケルのその後」

　訴訟の国アメリカで、マイケル・ジャクソンの死が、その後、もめないわけがありません。ジャクソンの父が当時の医師を民事で訴えました。

マイケル・ジャクソンの父は、ポップスターを最後に診た医師を不当な死であるとして提訴する意向であると述べた。ジョー・ジャクソンの弁護士ブライアン・オックスマンが、ロイターに述べたところによると、マイケルに投与された処方薬の量は、第二級殺人に相当するという。

「それは、六つのチェンバーしかない銃に六発の弾丸を充填したロシアン・ルーレットだ。」と彼は言う。

コンラッド医師は、犯罪捜査に属する計画性のない自己殺人の罪には当たらないとの嘆願を行った。ジョー・ジャクソンの訴訟は民事訴訟として、刑事裁判と並行して行われることになる。犯罪行為は実刑になるが、民事訴訟の主な目的は、マーリー医師への金銭的なダメージを狙ったものである。

「皮膚漂白クリーム」

　　オックスマン弁護士によると、マイケル・ジャクソンは6週間にわたって毎晩プロポフォールを投与されていたという。そして、それは無謀で、第二級殺人に相当すると言う。

スリラーやバッドなどのヒット曲でよく知られる歌手が昨年50歳で亡くなった。ロンドンで予定されていた一連の公演のリハーサル中だった。

今月初め、カリフォルニア司法長官エドモンド・ブラウンは、マーリー医師は芸能人の死亡について告訴されている間は診療行為をするべきではないと述べた。一方、彼の死亡後の捜査中に、大量の全身麻酔薬と無数の皮膚漂白クリームのチューブがジャクソンの自宅から見つかったことが判明した。

そのクリームは一般に、白斑の治療に用いられ、ジャクソンは色素がところどころ消失する白斑で悩まされていた。

　一見、謎の多い死のようですが、単純なものかもしれません。しかし、それを説明するのは至難の技かもしれません。

　過労死と薬剤副作用？

　予め計画された殺人であることが証明されることはないでしょう。しかし、薬剤の予期し得なかった過剰効果であったのか、何らかの基礎疾患による過労死であったのか。。。

　結論は年内には無理かもしれませんね。

「美しい歌はどこから」

少年のお話のつづき。。。

生まれながらの指の奇形が問題になるのは、小児期の成長にともなう拘縮や機能障害であり、それによって生活の質が損なわれるからです。合指症は、生後1年前後の早期手術するのが良いのは、その後の指の発達を促すためです。

少年の治療方針を決めるためには、その機能障害の程度を充分見極めなければなりません。

幸いにも、この6歳の症例は、中手骨と指関節が充分発達した多指症で、それぞれの指の骨の形や長さも良好で、指関節の可動域もほぼ正常のようです。物を握る動作は良好で、握力、神経学的機能も問題ありません。

それぞれの指に分布する血管が独立して走行しているなら、手術の成否は、互いに癒着した三本の指を分離したあとに、指先を養う血管の循環が保たれるかにかかっています。

付着した指を分離したのち、側面の皮膚欠損部に皮膚移植片を移植します。移植後に皮膚が壊死しないように微小血管の豊富な腹部の有茎皮弁が良いかもしれません。

うまく生着するには、幾日か、両手を固定しなければなりません。皮弁指の皮膚と移植片の皮膚の縫合が治癒してのち、次は切り離しと面倒な形成は、移植片の循環と色が期待通りであることを確認してからになります。

この症例にどのような手術が実際に行われたのでしょう。足の術後の映像はありますが、残念ながら術後の手の映像がありません。

　　　なぜ?

答えは簡単。きっと、メディアには明らかに出来ない秘密があったからです。彼の個人情報です。すなわち、少年が手術室を出てきたときには、両手はお腹にしっかり固定されていたのです。

手の手術と足の指の切除を、二人以上の術者で分担したとしても、わずか5時間でこなす中国の形成技術は驚くべきレベルであると言えるでしょう。

確かに。。。でも、わたしなら、余分な手足の指を一本も取り除かないのが最良の選択かもしれないと、両親を説得するかもしれません。

いつの日か、誰も聴いたことがない美しいピアノを奏でる日が来るかもしれません。どんなに多すぎても、

指は神からの美しい贈り物です。

かれはすでにモンスターではないのです。

「新しい手足」

さて、前項のつづき。。。です。

感想と若干の考察をしてみたいと思います。整形・形成の偉い諸先生方におかれましては、お茶でもしながら、ぶっ！って笑ってみていただければ幸いです。

外科医はおそらく、8本の足趾のうち外側の3本を切除し、余分な組織を除去したり骨の末端を削ったりして皮膚を縫合しています。ビデオで術後の仕上がりを見る限り、外側の中足骨は術前のまま温存されているようです。

手術の適応は慎重でなければなりません。術後の機能は生活のクオリティを得ることは容易ではありません。

少年は靴を見つける難しさを解決できたでしょうか。そんなはずはありません。

もういじめられることがなくなったきれいな足の形と引き換えに。長い期間痛みをともなうリハビリに耐えなくてはいけません。

医師や両親は、「新しい足」に合う靴を探さなければなりません。リハビリの段階や成長段階に応じたオーダーメイドの靴をたくさん必要とします。

中足骨は、歩く時の荷重で、折れやすく変形します。走るときはなおさら荷重がかかります。

遺伝的な突然変異だからと言って手術が必要とはかぎりません。

さて、ここでもう一度見直してみるべきなのは手の指です。まずは、手術の適応とタイミング。それから、術式の選択です。レントゲン写真からみて、中手骨型と言われるものです。それぞれの指が発声段階から分離しています。問題は、もともと三本の指（第3，4，5指）の合指症であり、これが手術を難しくさせています。

この症例には、どんな手術がよいとお考えですか？

Surgeon probably removed 3 lateral toes out of 8 toes and sutured skin after timing extra tissue or shaving some edges of bones : plastics and osteotomy.

As far as I can see postoperative finish on video, lateral middle (metatarsal) bones of his feet seem to be left conservatively as they used to be. The indication of operation should be determined carefully because of the difficulty in accomplishment of postoperative performance and quality of life.

Did he get rid of difficulties in finding shoes? Far from it!

He needs a long-term painful rehabilitation in exchange for getting so nice shape that he never be bullied any more.

Doctors or parents nevertheless have to find a pair of shoes fit for his ‘new feet’. A lot of order-made shoes with special size and/or form fit for every level of rehabilitation and growth stage.

　　Middle bones of feet (metatarsal) are likely to be fractured and deformed under the weight which is very great on walking or more on running.

The genetic mutations like this case of polydactilia ( congenital extra extremities) do not always need operation.

   　Now we’ve got to consider extra fingers again, first the indication and timing of operation, and then choice of method. X-ray shows this is metacarpal type of polydactilia where each finger is embryologically isolated.  

The problem is it is originally complicated with zygodactyly of three fingers (3rd,4th,5th), which makes the operation much more difficult.

足の指はいったい何本。

　びっくりしたのは言うまでもありませんが、さらにびっくりしたのは。。。だれかが、15本というと、15本に見えてしまう。

世界の有力メディアも間違えたのでした。

　画像のfourteen fingersがビデオナレーションではfifteen fingersに増えてしまっているではありませんか。あれ。

　じゃ。手術で取ったのは何本？それぞれ5本ずつ残したのは確かに間違いがないことでしょう。

インドとか他の国では、指が多いと、神様からの贈り物。「幸運のしるし」なんだって。近所の人たちが「一度触ってみたい」といって、やって来るみたい。

　Lee shan Pang was born with 15 fingers and 16 toes. A 6-year- old from China knows the problem of having. This week he underwent 5 hours surgery to remove extra digits. According to a local media, Lee suffered years of bullying with other children calling him a monster.

　Dr. An showed why the boy was born with extra fingers and toes but says genetic mutation or problems during pregnancy could be to blame.

　Dr. Chan Lee Gee said the surgery was though success. “We have successfully completed operation as anticipated.”

　The shape of these fingers and toes improved greatly. Parents are now planning  to enroll him in a lower school. Lee says he looks forward to fitting feet into a pair of shoes.

　ある県の小児科崩壊の危機が騒がれています。普天間あたりで。。。

 今日のお題は？  　休息のない職場。。。です。

　当直が見ておくよ。。。な～んて、誰がいってくれました? 　

　　このような軍人の「そうか」を連発していると、倒れる前に、みな死んでしまうかも。 　これを「軍人職場」と言います。やればやるほど、ほめたたえられます。死んでも、反省されることなく、「鑑（かがみ）＝手本」になります。 　

　こうした称賛と手本は、日本中どこでも同じようです。とっくに戦争に負けたのに、軍人教育と軍国主義がつづいています。 いったい。。。なぜ？ 　

　医師が足りないとよく言われます。。。 

　兵士をどんなに増やしても、「休息のない行進」に生き残る兵士はいません。

　10人行進しても生き残ったのが3人として、100人行進すると？ 　

　30人と答えるのが、軍人です。 

　そもそも無理な行進なら、生き残っていること自体が理解できないことですので、正解は０人かもしれません。何百人でも同じかも。

Dr Azim said: "Our findings clearly demonstrate that pregnancy is safe in women with history of successfully treated breast cancer. There is a wide perception in the oncology community that women with history of breast cancer should not get pregnant for fear of pregnancy increasing the risk of recurrence by means of hormonal stimulation. This meta-analysis strongly argues against this notion.

"Now we are refining the results by analysing subgroups to examine the effect of the timing of pregnancy – for instance how soon after a breast cancer diagnosis is it safe to become pregnant – and differences in survival according to the patient's age, lymph node status and so on.

"It is still common that patients are faced with incorrect counselling regarding pregnancy and the chances of future fertility following the end of breast cancer treatment and, thus, they are denied the chance of getting pregnant. Nowadays, there is a rising trend of delaying pregnancy to later in life and more young women are cured from breast cancer. So it is important to provide high level of evidence to help physicians in counselling these patients. This work may result in improving the quality of life of millions of young women who finish their adjuvant breast cancer therapy and want to get pregnant."

Dr Azim said there might be a number of explanations related to hormones or the immune system as to why pregnancy seemed to confer a protective effect on breast cancer survivors.

"It is well known that oestrogen is associated with breast cancer development. However, beyond a certain level, oestrogen exerts inhibitory effects on breast cancer cells. Laboratory experiments have shown that oestrogen and progesterone receptors located on 60-70% of breast tumours undergo apoptosis (programmed cell death) when exposed to high levels of oestrogen, which possibly mimics levels encountered during pregnancy. Furthermore, prolactin is elevated in pregnancy and there is evidence suggesting that women with high levels of prolactin have a reduced risk of breast cancer relapse." He added: "Nevertheless, hormonal changes during pregnancy are very complex, and the effect seen in this study could possibly be the result of the interaction of the different hormones rather than an action of a particular one by itself.

"Immunological theories could partially explain the possible protective value of pregnancy as well. It has been shown that foetal antigens* are expressed on the tumour cells of the mother. Thus, antibodies produced by the mother in response to these antigens, may act as a kind of tumour vaccination."

Dr Azim said that they had contacted all the authors of trials published after 1995 to get extra information on subgroups, and this would inform their further research.

He concluded: "Nowadays, fertility after cancer has become a top issue not only for patients, but also physicians. In 2006, the American Society of Clinical Oncology published guidelines that state that fertility issues should be discussed with patients before treatment – a recommendation we believe is of great importance."

いまだに、乳癌治療後に、妊娠や将来の出産について間違ったカウンセリングを受けているのが実情です。こうして妊娠の機会をなくしているのです。今日、妊娠年齢が高くなってきています。そして、乳癌年齢も若くなっています。こうした患者をカウンセリングするときに役立つ高いレベルのエビデンスを医師に提供することは重要です。この研究は、乳癌のアジュバントバントセラピーを終えたのちに妊娠を望む多くの若い女性の生活の質を高めることになるでしょう。

アジム氏は、なぜ妊娠が乳癌生存者にとって保護的な作用があるのかと言うことに関して、ホルモンや免疫と結びつけていろいろ説明することができるでしょう。

「エストロゲンが乳癌の進展に関係していると言うのはよく知られていますが、エストロゲンは、あるレベル以上になると乳癌細胞に阻害的な効果を及ぼします。実験研究では、エストロゲンとプロゲステロンレセプターが60－70％の乳腺腫瘍に見られ、妊娠中に起こるような高いレベルのエストロゲンの下でアポトーシス（プログラムされた細胞死）が起こります。さらに、プロラクチンが妊娠で上昇し、プロラクチンのレベルが高い女性では、乳癌の再発リスクが減少するというエビデンスがあります。しかしながら、妊娠中のホルモン変化は非常に複雑で、このたびの研究で見られた効果はおそらくさまざまなホルモンの相互作用の結果であり、特定のホルモンの効果によるものではないでしょう。」と彼は言う。

「免疫学的な理論から、妊娠による保護作用を説明できるかもしれません。胎児抗原が母体の腫瘍細胞に表現されているとも言われている。したがって、母体が抗原に反応して産生される抗体が、ある種の腫瘍ワクチンとして作用しているのかもしれません。」

　アジム氏は、1995年以降出版された臨床試験の著者らにコンタクトを取り、ほかの分類情報を入手したので、それを今後の研究に生かしたいと言う。

彼は次のように締めくくった。「今日、癌のあとの出産は患者にとってだけでなく、医師にとっても最重要課題でした。2006年のアメリカ臨床腫瘍学会はガイドラインを発表し、出産の問題は治療の前に患者と話し合っておくべきであり、我々が信じていることを患者に勧めるのが最も重要である。」

「聞いているみんながひっくり返った。」

ついこの間（3月25日）、欧州7カ国の乳癌学会で、面白い発表があったようです。

乳癌はエストロゲンで伸展、転移が促進されるというのがこれまでの常識。だから、乳癌になってから妊娠すると、癌で亡くなるリスクが高くなる。

そんな、単純な理屈って、どーよ。

ということで、ベルギーの癌研究所が1970年と2009年の臨床試験の症例を全部まとめて分析したらしい。

Barcelona, Spain: Women who have been treated for breast cancer can choose to become pregnant and have babies, without fears that pregnancy could put them at higher risk of dying from their cancer, according to a major, new study.

In a meta-analysis of 14 trials, presented today (Friday) at the seventh European Breast Cancer Conference (EBCC7), researchers from Belgium and Italy found that, not only was pregnancy safe for breast cancer survivors, but, in fact, it could improve their chances of survival. 

　Breast cancer is the most common cancer for women during their childbearing years. As women delay starting a family until they are older, and the survival from breast cancer has improved, increasing numbers of breast cancer survivors want to have babies after their cancer treatment has finished.

　Until now, it was unclear whether it was safe for them to do so, due to concerns that the hormonal changes associated with pregnancy, in particular the increase in oestrogen, could prompt the cancer to recur or become more aggressive.

　Dr Hatem A. Azim, Jr., a Fellow at the Department of Medical Oncology at the Institute Jules Bordet (Brussels, Belgium), and colleagues in Italy analysed results from 14 trials that had taken place between 1970 and 2009, involving 1,417 pregnant women with a history of breast cancer and 18,059 women with a history of breast cancer who were not pregnant.

　They found that patients who became pregnant following a diagnosis of breast cancer had a significant reduction of 42% in the risk of death compared to breast cancer survivors who did not get pregnant.

　ブリュッセル（ベルギー）にあるジュール・ボルデット研究所の医学腫瘍学の研究員ハーテム・アジムと、イタリアの共同研究者らは、1970と2009年に行われた1417例の乳癌の既往歴のある妊娠女性と18059例の乳癌既往歴のある非妊娠女性の１４の臨床試験の結果を分析した。

　それによると、乳癌と診断されたのちに妊娠した患者は、妊娠しなかった乳癌生存者と比較して死亡のリスクが42%も有意に減少していた。

オバマ大統領は、木曜日、医療保険法案の採択に対して国民が抱いている疑問を整理しようと、アイオア州を訪問した。木曜日にアメリカ議会上院は、停滞している医療保険改革法案に最後の検討を加えて最終修正案に合意し、火曜日にオバマが施行署名をすることになった。

そういうことですが、法案は私たち次第です。この法案により、保険会社は、入会する新しい被保険者と引き換えに守るべき幾つかのルールがあることが分かるはずです。すなわち、すべての人を誠実に扱い、すべての病人に治療を施すということです。（拍手喝采）

しかし、オバマと民主党議員は医療保険の改正の通過のために政治的空白をつくり、共和党は守りの立場に立たされています。

警察は議会のメンバーに対する恐喝と暴力事件を調査中です。選挙陣営にある議員事務所の窓にレンガが投げ込まれた。当局は議員に対する殺人予告について話し合うために会合を開きました、とスタイニー・ホイル議員は述べた。

「我々は、警察のトップと会うつもりだ。それと、同様に重要な関心を持っているFBIとも。」

2.2兆ドルの医療保険制度改革は、アメリカの40年で、最も画期的な医療政策の手直しであり、ビデオの大衆はお金のかかる政府による現行医療制度の乗っ取りだとして、来る11月の選挙で、議会における勢力を取り戻そうと叫んでいる。

楽しい幸せなペットとの生活は永遠には続きません。

いつか、それを失うことで必ず終わりがきます。

それが、ペットロス。

あなたの平均寿命、いくつか知ってる？

ぜったいバラしちゃだめよ。

そうね。人間の四分の一しかないの。

あるいは、それよりもっと短いの。

An enjoyable and happy life

with a pet will not last forever.

Some day or other

it inevitably comes to an end

by losing him,

say, pet loss.

Can you tell

what your average life span is?

Do you promise

you won’t let the cat

out of the bag?

Mark my words:

it’s only a quarter of human’s life, 

or maybe less.

with the job description (you know looking out for their best interest and yes it is protecting them).

When you own an animal you think on behalf of them, it kind of goes

Being informed is always a good thing before commenting and all I’m saying is that this machine may not be good for some dogs. The experience could mentally damage them and if you “get your dog in there and see if it likes it.” then you’re not protecting it.

I think that if someone decided to lock their dog in that machine they should have to watch and make sure that the dog is ok with what is happening to it.

And it is a shame, that people do not understand that DOGS ARE NOT CLOTHS.

Dogs are living, breathing animals and telling people, who do not want to wash their dog by machine, that they are “the people who stop progress” is NOT the best way to make them see the other side of the picture.

Also calling people hypocrites because they use washing machines for cloths and cars yet they are against this machine is the worst analogy you could have used.

Clothes and cars, as most of us know, are not living breathing organisms; whereas dogs are.

And COME ON of course when a child reaches a certain age they wash themselves but that is because THEY CAN.

Dogs don’t have thumbs or fingers to turn on water and wash themselves.

If you insult someone’s intelligence do it in a way that you don’t look or sound stupid.

Clothing, cars, humans, none of these are comparable to dogs when talking about washing them.

Like I said two of these are not living and another can communicate with the individuals who are in charge of them.

So why don’t you come on! and realize that just because many people would not use this does not mean they are ‘stopping progress.’
Yes we love pets and if people are too lazy to wash them than they should not get one.

　コメントする前によく理解することは大切だ。この機械はある犬にとっては良くないだろうと私は言いたいだけだ。これをやると犬に精神的なダメージを与えるかもしれないぞ。それにだ、自分の犬を中に入れて、気に入るかな～、どかなーなんて見てみようというのは、犬を愛護していないということなのだ。

もしも、あのような機械に犬を閉じ込めようというのなら、犬がどんなことになっているか、犬は大丈夫かきちんと観察して確かめなくてはいけないはずだ。

犬は洗濯物じゃないということが分からんのは トンデモナイ ことだ。

犬は生き物である。呼吸もしている。機械で犬を洗いたくないという者どもは、まさしく「進歩を阻止している」者たちだというだけでは、物事を違った側面から考察させる良い方法では「ない」！と、犬も言っておるぞ。

あ。そうだ。しかもだ、犬は人間を偽善者だと言っているぞ。衣服や車に洗濯機を使っていながら、この機械には反対している。これは人間が使う偽善によく似ておるわ。ってな。

衣服や車は、皆知っての通り、呼吸をしている生き物じゃ～ない。でも、犬は呼吸しているぞ。

それに、もちろん、よろしーか？子供がある年齢になると自分で体を洗うことができる。けれども、それは「自分で出来るYOU CAN」からにほかならないのである。

犬は、水を出したり自分で体を洗う指がないであろう。

お前たちが、もし誰かの知性を侮辱するというなら、せいぜい自分がトンマにみえないようにやってみるがよろしい。

衣服や車、人間、これらはみな洗えるが、洗うと言うことになると、犬と一緒にはできないのだ。

このうち二つは生き物ではない。人間の方はといえば、犬たちを飼っている人間と話し合いを持つことができる。ということで、よろしーか？この機械をたいていの人が使わないからと言って、「進歩を阻止している」ということにはならんぞ。

確かに、われわれはペットを愛している。でも、犬は好きでも洗うのは面倒という人は飼わなければよろしい。

Leave it to the ever-practical Japanese to try out every contraption without too much sensitivity towards the feelings or reactions of other people or other living creatures.

The dog washing machine seems to be a big hit among Japanese dog groomers, according to a Swedish TV report I just saw. The woman in the report says “I tried the machine out myself, to see that it was nice and safe for Mitzuho (the dog).”…

他の人間や動物の感情や反応に無関心で、カラクリ発明を試してみたいという、根っからのプラクティカルな（やってみなければ気が済まねー）日本人には、やりたいようにやらせておけばいいのさ。

この前、スエーデンテレビをみてたら、イヌ洗濯マシーンは日本の犬美容師の間ではビッグヒットらしいぜ。女性レポーターが言ってたぜ。「ミズホちゃん。だいじょうぶかな～って。わたしも試してみまぴた～ん」。。。だってよ。（ミズホって犬のことらしいぜ。）

This machine is a cruel way to capitalize on the laziness of pet owners.

If you are too lazy to properly care for your dog, or too cheap to take it to a groomer to be washed, then don’t have a dog. In our busy lives, our pets don’t get as much attention as they should to begin with.

Giving your dog a bath should be a comforting, interactive experience for the animal.

Even if they hate the bath, having someone’s hands on them gives them a feeling of security they cannot possibly get while locked in a scary machine.

There is also no control over where the water goes, such as in the ears, where an infection can develop.

Another benefit of hand washing your dog, is you have the opportunity to feel your pet all over, to discover things like hair mats, swellings, bumps or painful areas you would not have known about otherwise.

Animals don’t need high-tech. Keep it simple.

この機械は、ペットの飼い主たちの怠慢に投資する残酷なやり方です。

自分のペットを気遣えない怠け者で、犬美容師のところへ連れて行って洗ってもらうのが安上がりなら、犬なんか飼わないように。忙しい人間生活の中で、ペットたちは当然受けるべき関心を払われていないのです。

入浴させるのは、犬には気持よい相互体験になるはずです。

たとえ風呂を嫌がっても、手で触れることで、安心感を与えることができます。怖い機械に閉じ込められると、それは決して得られません。

水がどこに行くかコントロールできません。例えば、耳の中とか、感染症が起こることもあります。

手で犬を洗うもう一つの利点は、ペットの全身に触れることができるということです。毛のもつれ、腫れもの、こぶ、痛がるところといった、手触れなければ気がつかないものを見つけることができます。

動物にハイテクは不要です。シンプルがいいのです。

わたしの可愛いフィービーちゃんにはダメっ！毎回、面倒でも、手であらってあげるわ。それでいいの。この洗濯機は、たいていの人が、自然と触れ合っていないという一例です。あー。さみしい・さみしいわ！！！

ついでに一言。わたしの「可愛いフィービーちゃん」、70ポンド（26Kg）のビットブル犬なの。この子をお風呂に入れるの、チョウたいへ～ん。でも「わたし気にならないわ！」。

この機械を発明した人に言いたいです。「あなたはペットも子供も持つ資格なんかないわ！」
