医師不足：新臨床研修制度導入…大学医局「人手不足」　医療機関への派遣中止８割
　新人医師が２年間の研修先を自由に選べる新臨床研修制度が導入された０４年度以降、大学病院の医局の約８割が人手不足などで地域の医療機関への医師派遣を中止・削減したことが、日本医師会の調査で分かった。派遣を受けられなくなった医療機関の６割以上が診療制限や診療科の閉鎖に追い込まれていた。大学から一般病院へと医師不足が広がった過程をデータで裏付けたのは初めて。【清水健二】
　今年３-５月、大学病院の１８２１医局を対象に実施し、１０２４医局から回答を得た。その結果、０４年４月以降、大学から地域の病院への医師派遣を中止したり、派遣数を減らした医局は７７％に達した。うち７８％の医局が「臨床研修制度による人員不足」を理由に挙げた。
　医局が医師を引き揚げた医療機関は３００３施設に上り、このうち１７％が診療科を閉鎖、４５％が外来のみの診療や診療時間短縮などの制限に踏み切ったという。診療科別では産婦人科、内科、リハビリテーション科の順に、引き揚げの割合が高かった。
　地域別に見ると、人口１０万人当たりの医師数の下位９県（埼玉、茨城など）で減らされた派遣医師数は一病院平均０・２８人。全国平均（０・２２人）より約３割多い。
　分析した日本医師会総合政策研究機構（日医総研）は「大学病院の人材配分システムが機能しなくなったことが医師不足を顕在化し、地域間格差を広げた」と指摘している。
　新人医師は２年間で内科、外科、小児科など各科を回って総合的な診療技術を学ぶことが義務付けられた。研修先として待遇のいい大都市の病院に人気が偏り、若手の大学病院離れが起きている。
以前から問題ありだったんだよ！
