Inflammatory abdominal aortic aneurysm

○5% to 10% of all cases of AAA
○"inflammatory AAA": Br J Surg. 1972;59:609–614
the triad of gross pathologic findings in inflammatory AAA: (1) a thickened aneurysm wall(0.5 to 3.0 cm)
(2) intense perianeurysmal and retroperitoneal fibrosis
(3) extensive adhesions involving surrounding organs
○“chronic periaortitis.” : (1) inflammatory AAA, (2) idiopathic retroperitoneal fibrosis, and (3) a combination of the 2 disorders.
○Differential Diagnosis
inflammatory AAA=AAA with intense periaortic and retroperitoneal fibrosis and inflammation

・Systemic vasculitis.
Takayasu aortitis
Temporal arteritis
Cogan syndrome
・Other nonrheumatic diseases can cause aortic aneurysms but without periaortitis)
Marfan syndrome
Ehlers-Danlos syndrome
cystic medial necrosis
infections (salmonella, syphilis)
