Chest. 2006;129:1210-1218: The concept of deescalation therapy is emerging as an effective strategy for the management of "Ventilator-Associated Pneumonia" and other serious infections（Chest 2002;122,2183-2196）.

"Therapy escalation" was defined as the switch to or addition of a drug class or classes with a broader spectrum or additional coverage. Deescalation was defined as a switch to or discontinuation of a drug class resulting in a less broad spectrum of coverage(=This concept entails the early implementation of broad-spectrum empiric coverage followed by a regimen tailored according to susceptibility findings).

免疫抑制療法中の肺炎であれば、緑膿菌はカバーするか？誤嚥の要素を考慮するか？MRSAに対するバンコマイシンを含めるか？
吸痰の培養結果がでれば抗菌薬を絞る。

感染症学的には人工関節感染には人工関節の除去が望ましいが、できない場合に（以下のような用件を満たせば）、Prolonged Suppressive Antibiotic Therapyが行われることがある。

（(1) prosthesis removal is not possible
　(2) the pathogen is relatively avirulent
　(3) the pathogen is exquisitely sensitive to an orally absorbed antibiotic
　(4) the patient can tolerate an appropriate oral antibiotic
　(5) the prosthesis is not loose

・Quorum-sensing system （クオラム・センシングシステム（集団密度感知制御システム（quorum :（会議の）定足数; 選抜者の集団））：
バイオフィルム内でコミュニティ（Slime City:スライム・シティ）を形成している細菌群は"autoinducer"とよばれる分子を介して細胞間で情報伝達を行っている。

・マクロライド少量長期投与の分子標的として、Quorum sensing機構によるBiofilm形成プロセスが想定されている。

(NONPHARMACOLOGIC THERAPY)
• Supplemental oxygen
• Ventilatory support if needed
• Prompt thoracotomy if pneumothorax develop
• PCPの治療
(Regimen 1 バクタ) Trimethoprim-sulfamethoxazole (20 mg/kg trimethoprim and 100 mg/kg sulfamethoxazole qd) PO or IV
(Regimen 2 ベナンバックス) Pentamidine (4 mg/kg IV qd)
(呼吸不全時　ステロイド) Either regimen with prednisone (40 mg PO bid):
1. If arterial oxygen pressure <70 mm Hg
2. If arterial-alveolar oxygen pressure difference >35 mm Hg
3. Dose tapered to 20 mg bid (twice daily) after 5 days and 20 mg qd (every day) after 10 days
• Therapy continued for 3 wk
• Alternative therapies available for patients unable to tolerate conventional therapy:
1. Dapsone/trimethoprim
2. Clindamycin/primaquine
3. Atovaquone

追記　足して50（> or = 50 positive cells per two slides）については、骨髄幹細胞移植患者で、足して50以上をHigh-grade cytomegalovirus antigenemiaと定義し、この群では約半数でサイトメガロウイルスdiseases感染症を呈すると報告されている。low gradeでは4%（In, High-grade cytomegalovirus antigenemia after hematopoietic stem cell transplantation.
