・喫煙・HLA-DR (SE)・CCP抗体間には相互作用ある傾向あり。
・HLA-DR (SE)とPTPN22-1858Tは抗CCP抗体産生について相互作用する傾向有り。

対象
・Epidemiological Investigation of RA (EIRA) study（喫煙情報有り）
・the North American RA Consortium (NARAC) study
・the Dutch Leiden Early Arthritis Clinic study
(in total, 1,977 cases and 2,405 controls)

方法
（１）Rothman法：リンクしていない二つのローカスの遺伝子相互作用解析：departure from a additive model: 古典的なリスクファクター間の相互作用モデル：AP = attributable proportion due to interactionを計算して評価
（２）Logistic regression model　（Multiplicative or stastical interaction）
（３）Gregersenの方法：二つのリンクしていないローカスにおけるdeviation from independence of penetrance Linkage disequilibrium (LD) statistic

（１）PTPN22, SE, SmokingはCCP(+)RAではどのコホートでも関連するが、CCP(-)RAとの関連はNARACでのみ有意であった。
（２）遺伝子間相互作用
・Rothman法：全ての集団でPTPN22, SEは相互作用を呈していた。PTPN22, SEの機能からすると生物学的には抗原提示細胞と　T細胞間の相互作用に相当すると考えられる。
・Multiplicative相互作用：個々の集団ではパワー不足で有意さはでないが三つをまとめて解析すると有意な結果であった。
・LD法：EIRAとNARACでは有意な相互作用が検出されたが、Leidenサンプルでは有意さ得られなかった。
（３）遺伝子・環境要因相互作用-I
・PTPN22と喫煙の相互作用無し
（３）遺伝子・環境要因相互作用-II
・PTPN22, SE, Smokingをまとめて解析：CCP(+)RAではそれぞれの組み合わせで有意な結果が得られる組み合わせがあった〜これは層別解析？。

追記　epistasis (GxG) や GxE interactions解析

603例のRAを13年間フォローした結果、リウマトイド因子（−）ではステロイド服用しても心血管イベントは増加しなかった（irrespective of cumulative exposure総投与量, average daily dose１日平均用量, and timing of exposure to glucocorticoidsいつ頃投与されたかに関係なく）。しかし、リウマトイド因子陽性RAではステロイド使用は心血管リスクを上昇させた。

16,788例のRAを対象に肺炎（入院）のリスクを検討したところ、ステロイド使用は用量依存性に入院加療を要する肺炎のリスクを上昇させた（1.7 [1.5-2.0]：5mg/day HR 1.4 ：>5-10 mg/day HR 2.1：>10 mg/day HR 2.3）。
レフルノミド＝HR 1.2 [1.0-1.5])：エンブレル＝0.8 [0.6-1.10])：アザルフィジン＝0.7 [0.5-1.0]) 。一方、レミケード、ヒュミラ、リウマトレックスはほぼ0．

治療：ステロイドは15mgから開始して４ヶ月かけて減量・中止（４週毎に可能であれば10 mg, 5 mg, and 2.5 mg、中止）。
インフリキシマブは3 mg/kg で0, 2, 6, 14, and 22週に点滴静注
結果：52週において再燃再発のない患者はプラセボ群で37％、レミケード群で30％

（PMRには２型有り、1-2年の治療でステロイドを中止できる軽症群、および慢性で再発をおこしステロイド治療を数年あるいは永久に要する難治性群。今回の試験は後者におけるレミケードの有用性の可能性を否定するものではない）

Johnson & Johnson has proposed that Britain’s national health service pay for the cancer drug Velcade, but only for people who benefit from the medicine, which can cost $48,000 a patient. The company would refund any money spent on patients whose tumors do not shrink sufficiently after a trial treatment.
