研究者も医師も企業や社会との関係を考えなければならない

・まずは近況報告から。なんせ、左腕が痛いので本日１日安静。お昼ごはん食べにだけ外に出ました。安静にしていると痛みは楽なのですが、なんと風呂桶（古い言い方？今は、バスタブ？）洗っただけで腕がしびれてきました。

・本日『脳科学の真実』（坂井克之。河出ブックス。２００９年１０月）という本を読みあげました。非常にお勧めですね。著者があとがきで書いていますが「さまざまな視点から脳科学と社会の関係について考えてみた」と書いています。まさにその通りですが、それだけでなく、この中で、科学的な考え方や、研究の在り方、評価の仕方等科学自体の在り方も問題提起され、「現代科学論」的な側面もあります。

・バイ・ザ・ウエイ、他のDr.のブログみていたら、東京日和＠元勤務医の日々というsiteで、「パンドラの函：製薬企業と医師の関係に見直しが入る・・・」という記事がありました。このブログでも何回かとりあげた話題です。リンクしていますから、お読みください。ただ、この標題の「パンドラの函」とありますが、なぜそうなのか私には理解できません。わたしは、べつに「パンドラの函」という感じは持っていないのですが．．．

・あと、本日したことは、新海誠の『雲のむこう、約束の場所』というＤＶＤを見ました。私は、『秒速５センチメートル』を最初に見て、次に『ほしのこえ』をみて、これが３番目です。どの作品見ても、「ああ、新海誠」という感じですね。色使いや、画面の分割、ショットの角度、光、電車や踏切、そしてそのストリーのせつなさ等。今回、９０分作品でちょっと見るのがしんどかったですが。世界にたった一人という状況を想像してみて、恐怖を感じました。（今まで、そういうＳＦやマンガはいくつも見た気がするのですが、年をとったせいか、恐かった．．．）あと、飛行機が高度を上げていくシーンがすごかった。（ああ、言語表現の貧困を感じる）ただ、ＤＶＤのジャケットは、もっと違う絵が適切ではないかと思ってしまいました。（この記述に関して、「オッサン、何書いてんねん？」と思った人は、ＤＶＤを借りるか、買って、まず、見ましょう）

　コメントありがとうございます。おそらくご存知ないかもしれませんが、日本の製薬企業のプロモーションコードを忠実に守ると、今のような形での営業はできないはずです。
　そして、熱心なのは内資系の企業ばかりです。外資系の製薬企業はそこまで熱心ではないと思います。
　そんな中で、臨床研究に対してメスが入ると、内資系の研究支援の名の元に行われる、様々な営業活動ができなくなります。日本の製薬市場は5：5で内／外資ですが、新薬の上市は最近は3：7です。
　近年、ピカ新とよばれる新薬をほとんど出せない大手内資系の企業にとって、今年の春の改定で長期収載品の価格改定と今回の資金提供の見直しは、営業に影響がされ、内資系製薬企業にとってダメージが大きい可能性があります。
　パンドラの函というのは、ちょっと違う使い方になってしまっていますが、おそらく大学の研究室に足げく通うMRさんが研究資金をもたらす事はどんどん減ると思います。なぜなら「減収」が確実なので、販促品や便宜供与を削るしかないのです。

製薬会社を擁護するつもりはさらさらありませんが、ここ数年の統合合併で個人的にも親しくしていた（便宜供与なしで）MRの方が何人も会社を替わったり転職したりされました。

そういう意味では、勤務医というものはよくも悪くも奴隷医であるなあ、と感じます....

別に私は、製薬メーカーを敵視しているわけではありませんし、そこで働くＭＲさんには、同情することもあります。藤沢と山之内が合併する時、在宅酸素療法をしている部門の職員さんは、本当に気の毒だと思いました。自分たちがどうなるか、本当に直前まで分からなかったのですから。
ＭＲさんを、私が、どう見ていおり、何を期待しているかは一度記事にしたいと思っているところです。（なんか、とっても長い記事になりそう）
