ヘルペス後神経痛に、カプサイシン・パッチ

・ＦＤＡがヘルペス後神経痛の治療薬として、Qutenzaという薬（ a patch containing 8% capsaicin）を承認したとのことです。現在私の患者さんで、三叉神経領域の帯状疱疹の方がおられ、痛みのコントロールをどうしようか悩んでいるところです。カプサイシンが効くのですね。キムチ一杯食べたら、ヘルペスの痛みが改善したりして．．．（あくまで、冗談ですよ）このことの詳細は、↓

超早い、「以下日記」

本日は７時１０分に病院へいって、病棟の回診後、９時より外来でした。患者さん、多かった。病棟当番のDr.にも外来を手伝ってもらいました。本日一番の「大物」は、大動脈解離（解離性大動脈瘤）。診断までにかなりの時間を要しました：まず、心電図と採血。そして平面胸部レントゲン撮影。そして単純ＣＴ。それから造影ＣＴ。造影ＣＴとるのにちょっと難儀しました。本日は、当院の放射線技師がおらず、他院から応援の技師さんでしたのですが、当院のＣＴでは、「造影してＣＴをとる「タイミング」がわからない」と・・・仕方ないから休んでおられた当院の技師さんを呼び出しました。呼び出してまで、造影ＣＴをとるかどうか悩みましたが、臨床的に大動脈解離を強く疑っていたので、呼び出しました。それで、ビンゴでした。解離がわかり、天下のＫ中央病院に紹介しました。当院のような中小病院は、土日、休日の時の人では不足しており、というか、平日も不足しており、早急な検査をして診断を迅速につけるというのが、困難だったことが多々あります。ほんま、中小病院はつらいよ。

・１４時前にやっと病院出て帰宅。夕方三女（中学生）と岡山市へ。三女が、「都会じゃ、ビルが大きい」と、カッペまるだして感心しておりました。大阪行ったことがあるのに。本屋に行って１時間弱時間をつぶし、１８時に神戸に行っていた配偶者と次女に合流、『鮮寿』というお店に行って、夕食を食べました。久々においしい料理と、おいしいお酒で、ほろ酔い、とても気持ち良い食事ができて幸せでした。本屋では、諸星大二郎の新刊も買えて、ラッキー、寝床で読みます。鮮寿で飲んだ、「千寿」という日本酒がうまかったので、こんど見つけて買おうと思います。あと、「桃の里」というお酒もおいしかったな。

よく拝読させていただいてます。毎回興味深い話題をありがとうございます。

当院でもカプサイシン軟膏を作って、患者さんに実費で分けてます。
頻回に外用するのがいいようです。気に入ってくれる人も多いようです。確か0.125%位だった思いますが、刺激が強すぎるときは白色ワセリンで薄めて差し上げます。三叉神経領域には出したことないですね。論文のパッチも８％でも非常に薄く塗ってあるのでいいのでしょうが、キツそうですね。パッチになっていた方が目に入ることがないでしょうね。

カプサイシン軟膏のstockを作る時、（クリニックにはドラフトはないので）マスクしていても、流涙と鼻水で大変です。

どうしようもない結節性痒疹やビダール苔癬にもときどき使用します。
