労働安全衛生よりみた病的賭博

・先日ある種のクスリの副作用で，病的賭博が起こることを書きました．その論文を読んで，二つのことを考えました．一つは，ある種の化学物質でも病的賭博が起こりえるのではないかと．ＴＯＸＮＥＴでpathological gamblingをいれてみましたが，そういう物質はなさそうです．でも，研究が進めばあるかも．

・もう一つ考えたのは，クスリや化学物質とはなれて，職場における病的賭博の問題．これは，労働安全衛生の対象にしなくてはならないのでは．．．私には，メンタルヘルス関係でのターゲットの疾患は，まず，うつ病，次にアルコール依存というイメージがありました．そこに病的賭博も加えなくては．．．以下の様な論文がありました．

  

産業保健における病的賭博－借金問題をかかえる従業員－

福田秀二． 産衛誌 46巻，2004．A35～Ａ36．

抄録はありませんので，この中に書かれていることを，つまみ食い的に書いてみます．

・・・退職者や自殺者の中に賭博による借金問題が含まれていることを時々耳にする．そうした事例では，職場関係者はもちろん産業保健スタッフも，賭博による借金苦を抱える者を治療の対象だと考えることはまずない．

 

  病的賭博者は，賭博をしていないときは，仕事中毒またはむちゃくちゃ働く労働者であり，本当に働きすぎになるぎりぎりの線まで働く．

 

  病的賭博は治療を要する病態である．しかし，そうした理解は本邦ではまだ浸透していない．臨床医でさえ，病的賭博者が受診しても，病的障害だと診断するのを躊躇しやすい．賭博による結果を自業自得とし，患者を単に道徳欠如者として片付けるやりかたは，ちょうど30年前のアルコール依存者にたいする臨床家の態度に似ている．

 

・・・抗うつ剤投与で改善をみる場合もあるが，確実ではなく，アルコール依存からの回復に用いられると同様の自助グループ参加が有効である．・・・退院後の支持基盤としてGamblers Anonymous(GA)への導入は最重要課題とされる．

 

  病的賭博は，本邦ではまだ臨床医の啓蒙さえも進んでおらず，本格的研究は手つかずのままである．

 

  病的賭博はギャンブル依存症と換言できる．産業保健スタッフとして，まずは，精神障害としてのギャンブル依存症の存在を認知することによって，借金問題をかかえる従業員を画一的にメンタルヘルスの問題から切り離してしまう誤りを防ぐことができる．

そうだと思うのですが。今、興味あるのは、そういう母親が逮捕されたとき、ただ単に罪に問われているだけなのか、精神科の受診をうけているかです。
