臨床医学に環境・労働の観点を

2005年の「クボタショック」以来アスベストが社会問題となりました。アスベストは、さまざまな問題を提起していますが、ひとつに、現在の臨床医学がいかに職業・環境因子を軽視していたかが明らかになったと思います。現在の臨床医学に欠けている職業・環境因子の疾病へのかかわりを考えていきたいと思います。私が一人情報発信するのではなく、問題提起をしてそれにこのブログを見てくださった皆さんが応えてくださることにより知識・情報を集積したいと考えています。よろしくお願いいたします。

・私の持っているハリソンの内科学書は　１５版２００１年発行で古いのですが、INFLUENCE OF ENVIRONMENTAL AND OCCUPATIONAL HAZARDS ON DISEASEという項はいろいろ良いことを書いていると思います。

・ちょっと、抜粋してみますと

・・・careful histories have identified occupational factores as etiologic in more than 10% of all admissions to general internal medicine wards in hospital, with even higher percentages when the primary illness is either respiratory or musculoskeletal.

More than 15% of patients seen in one study conducted in a primary care clinic expressed the opinion that their health problems were work-related, and 75% of this subgroup of patients reported exposure to one or more recognized toxic agents.

・結局日常臨床では見落とされていますよ、ということです。

・職歴や環境因子をキチッと聴くことができれば良いのですが、忙しい日常臨床では難しいし、そもそも職歴を聴いてもピンとこない＝仕事の内容や使っている物質、労働環境がわからない、ということがあると思います。偉そうにブログの題名をつけている私も、分かりません。

Unfortunately, few physicians have more than rudimentary training in environmental and occupational medicie.

と書かれております。

・そういった中、ハリソンはScreening questionsとして以下の質問をするようにアドバイスしています。

・What kind of work do you do?

・Do you think your health problems are related to your work, home, or any other particular environment or exposure?

・Does the timing of your symptoms have any relation to being at work or at home or to any other particular exposures or activities?

・Are you now or have you previously been exposed to dusts, fumes, chemicals, radiation, or loud noise?

・とくに赤字のところは、本文にもイタリックで強調して書かれていました。外来で、「その症状は仕事や環境に関係しているとお考えですか？」と「健康物語」を聴くのが大切だと思います。
