当直医、妊娠見逃し死亡

またしても残念な医療過誤が発生した。

　産婦人科医･外科医ならたいしたことない問題なのかもしれない。

　しかし、内科医にとっては一番嫌な条件。
　若い女性の腹痛だ。

　問診で「妊娠している可能性がありますか？」と状況によってはナイーブな質問をしなければならないうえに、診断がついたところで対処が難しい。
　変態新聞が地域医療を破壊した引き金にもなった産婦人科関連だ。

　内科の中でも腹部症状に対して比較的眼力のある消化器外科。そして、触診、レントゲン検査までやっている。
　それでいながら「見落とした」と非難されるという不幸な転機。

　私も女性の腹部症状はもちろんたくさん見てきている。
　そのうち妊娠に当たったのは１回。17才の女性だった。

　そして、それ以上に困難な症例に当たったことがある。
　１年目の研修医だったその頃、「当直医マニュアル」なるものを頼りに当直のバイトをしていた。
　夏の夜も更けてきた頃、腹痛の女性が電話もなく直接来院した。

　私は汗だくになりながら、
　「妊娠反応は陰性ですけど、腹部の症状が強いので念のためCTをとらさせてください」
と、患者に提案した。患者は非常に素直に頷いてくれた。学生の知識しかなかったのであるし、妊娠した女性を見たこともなかった。しかし、どうしても気になったから。

　そのときは救急も「やってる」民間病院だった。
　自宅待機のレントゲン技師に電話すると、非常におっくうそうに嫌々ながら電話に出て、それでも出勤してくれた。
　そして、腹部CTをとる。子宮の近くにヘチマのたわしのようなものがある。
　当時の知識で多嚢胞性卵巣と診断した。腹部症状も強いし熱も出ている。緊急の治療だけでも産婦人科医にお願いすることにした。
　私は20km離れた2次救急病院に電話した。そこにしか産婦人科医がいなかったのである。

　産婦人科医は門番のように私の受け入れ要請を拒んだ。しかし、私がまだ３ヶ月の研修医で診断にも自信がないと言うと、「それならば」と一転受け入れてくれた。
　このとき味わった心細さが尾を引き、若い女性の腹部症状に極端に恐れを抱くようになった。今でも出来れば診察を避けたいと思っている。

　記事を見たとき、そのことを思い出してしまった。消化器外科の先生も心細さを抱いていたに違いない。

東京都世田谷区の長谷川病院（長谷川利治（はせがわ・としはる）院長）で２００５年４月、救急搬送された町田市の女性＝当時（２６）＝が約９時間後に死亡する事故があり、警視庁捜査１課などは２９日、適切な検査をせず、女性の子宮外妊娠を見逃したとして業務上過失致死の疑いで、当直だった男性医師（５０）を書類送検した。
